Coming home from hospital or a care home is usually a relief, but it can also be a surprisingly delicate moment. Discharge often happens quickly, a loved one's needs may have changed while they were away, and, very often, their medication is different from when they went in. Getting this transition right matters enormously, both for their safety and for their peace of mind.

The good news is that a well-managed return home is exactly what we're here for. At Edyn, we treat the journey back home as carefully as any part of the care itself, coordinating closely with everyone involved so your loved one comes home to safety, calm and continuity. And this is true whether your loved one is coming home after a hospital stay or moving home from a care home: both are transitions we handle with the same care. Here's what happens.

Planning a safe return home

As soon as discharge is on the horizon, our team gets to work planning for it. We request the discharge information from the hospital, family or your loved one, and we work with everyone involved to make sure the return is safe and well-organised. Wherever possible, we aim to give good notice so everything is ready, and where a discharge happens suddenly, we manage it flexibly, so your loved one is never left waiting to come home.

Getting medication right: a safe, coordinated handover

This is the single most important part of any discharge, and it's where careful communication really counts.

Hospital stays very often bring changes to medication: new medicines started, dosages adjusted, or existing ones stopped. If those changes aren't captured accurately and passed on clearly, it's one of the biggest risks at this stage of the journey. So we take it extremely seriously.

Our Care Managers communicate directly with the professionals involved, including the discharge nurses at the hospital and any other care staff coordinating your loved one's care to confirm exactly what has changed and why. We then make sure your loved one's carer at home has complete, up-to-date medication information, so that nothing is missed, duplicated or given incorrectly once they're back.

Alongside this, the carer carefully records the details on their return: noting any new medications (along with the prescription and discharge notes), flagging any doses that have changed or medicines that have been stopped, and uploading the discharge documentation so there's a clear, accurate record. It's a thorough, joined-up process, and it's how we make sure the return home is not just smooth, but safe.

Reviewing what else may have changed

Medication aside, a hospital stay can change other things too: a loved one's mobility, their memory or cognition, or the everyday risks around them. There may also be new recommendations from the hospital team to take on board.

So before normal care simply resumes, our Care Manager reviews the discharge documentation and these changes carefully. Where needed, we'll carry out a reassessment and update the care plan, so the care reflects your loved one's needs as they are now, rather than as they were before they went in. Settling back in, with a familiar face

For someone who's been unwell and away from home, few things are more reassuring than returning to their own surroundings, and to a carer they know and trust. Their carer welcomes them back to a home that's ready and comfortable, and provides attentive, one-to-one support through what can be a vulnerable recovery period.

They'll also keep a close eye on how your loved one is doing in those first days home, noting any changes in their condition or mood, and reporting anything of concern promptly, so that recovery stays on track and any new needs are picked up quickly.

Moving home from a care home

Not everyone comes to Edyn from a hospital. Increasingly, families choose to move a loved one out of a care home and back into their own home, to enjoy the comfort, familiarity and one-to-one attention that live-in care provides. It's a wonderful step to take, and, just like a hospital discharge, it's a transition we manage with real care.

When someone moves home from a care home, our Care Managers liaise closely with the care home's staff, including their managers and nurses, to ensure a safe, seamless handover. And, exactly as with a hospital discharge, medication is the priority: we confirm precisely what your loved one is taking, along with any recent changes, directly with the care home team, so their carer at home has complete and accurate information from the very first day. We'll also gather their care records and take time to understand their routines, preferences and needs, building all of it into a personalised care plan for home.

The result is a smooth, well-coordinated move, and a loved one settling happily back into the place they know best, with dedicated, one-to-one support all their own.

When needs have changed more significantly

Occasionally, a hospital stay changes what someone needs more substantially,  perhaps a higher level of support than before. If that's the case, we'll talk it through with you openly and adapt the care accordingly, so it continues to fit your loved one perfectly. And while we always aim for the same carer to return after a hospital stay, if a different carer steps in, you can be assured they'll be fully briefed and ready to provide the same standard of care.
