A hospital admission almost always comes out of the blue, and it's a worrying time. On top of concern for your loved one, all sorts of practical questions can suddenly arise, and if they have a live-in carer, one of the first is likely to be: what happens to their care now?

It's a very reasonable thing to wonder, and the reassuring answer is that we've planned for exactly this. Hospital admissions call for flexibility, and we have a clear, calm process to handle them: keeping your loved one safe, keeping everyone informed, and taking care of the details so it's one less thing on your mind. Here's how it works.

The moment of admission

If your loved one is admitted to hospital, their carer contacts our office straight away with the details. From there, our team takes over the coordination: we record the admission, alert the right people, start liaising with you and the hospital, and begin thinking ahead to a safe return home.

In other words, from the very first moment, you're not managing this alone: our team is right there alongside you.

The first 48 hours: ready and waiting, at no extra cost

Hospital stays are often short, so for the first 48 hours we'll usually keep your loved one's carer at the home, ready to welcome them straight back if they're discharged quickly, and on hand to help with discharge planning.

Importantly, there's no standby charge during this initial 48-hour period. It simply gives everyone a little breathing room, keeps things ready, and ensures a smooth, safe return if your loved one comes home soon.

If the hospital stay continues

Where an admission lasts longer than 48 hours, we'll talk things through with you and agree the best arrangement. There are two main options, and your wishes guide the decision.

Standby

Here, the carer remains at the home, keeping everything ready for your loved one's return. They'll look after light household upkeep: things like laundry, fresh bed linen, keeping the property secure and checking the fridge, and help with discharge planning, so that the moment your loved one is ready to come home, everything is calm, clean and prepared for them.

Active hospital support

Sometimes, the greatest comfort is a familiar face in an unfamiliar place. With active hospital support, your loved one's carer regularly visits them in hospital, providing the one-to-one support they know and trust: help with personal care, eating and drinking, mobility, gentle reassurance, and being present for assessments and discharge meetings.

For someone who may be frightened or disoriented in a busy hospital, having their own carer beside them can make an enormous difference.

We'll help you decide which arrangement is right for your situation, and in keeping with our commitment to transparency, we'll always explain any associated charges clearly and agree them with you in advance. Whether you'd like the carer to remain on standby beyond the first 48 hours is entirely your choice.

Planning the journey home

As soon as discharge is on the horizon, we work with the hospital, with you, and with your loved one to plan a safe and well-organised return. Wherever possible, we'll aim to give good notice so everything is ready, though we know emergency discharges happen, and we handle those flexibly, case by case.

One honest note: while we always do our utmost to maintain continuity, we can't absolutely guarantee that the very same carer will be available to return after a hospital stay, particularly if it's been a long one. If a different carer does step in, you can be assured they'll be fully briefed on your loved one's needs and ready to provide the same high standard of care.

Settling back in at home

When your loved one returns home, we don't simply carry on exactly as before — because a hospital stay often changes things, and their care needs to reflect that.

Their carer updates us on the return, and our Care Manager reviews the discharge information carefully: any changes to medication, mobility, memory or risks, and any recommendations from the hospital team. Where needed, we'll carry out a reassessment or update the care plan before normal care resumes, to make sure everything still fits your loved one's needs and keeps them safe. It's a thoughtful, careful restart, never a rushed one.

Throughout a hospital admission and the return home, your loved one's safety and wellbeing are our absolute priority. If any concern ever arises: about their safety, a safe discharge, or their wellbeing, we act on it immediately and appropriately, through our established safeguarding processes. Nothing matters more to us.
